CIN is categorized by risk for developing cancer with CIN1 at lowest risk, up to CIN3 at highest risk.
Scotland has an established national cervical screening program and in 2008 began a national HPV vaccination program for girls aged 12 and 13, with a catch-up program for girls up to age 18. The HPV vaccination protects against human papilloma virus 16 and 18, which are responsible for 70 percent of cervical cancer. The study appears in the April 3, 2019 British Medical Journal (BMJ).
The study analyzed the vaccination and screening records for 138,692 Scottish women born between 1988 and 1996 who had a screening test at age 20, the age women in Scotland are first invited to undergo cervical screening. The records analyzed included women who did not receive the HPV vaccination born between 1988 and 1990, women who were eligible for the catch-up vaccine at age 14-17 born between 1991 and 1994, and women routinely vaccinated at age 12-13 who were born between 1995 and 1996.
In addition to the 89 percent reduction in CIN3, there was a 88 percent reduction CIN2 and 79 percent reduction in CIN1. The researchers also found that the younger the age of vaccination was linked to increased effectiveness of the vaccine, with women vaccinated at age 12-13 having an 86 percent reduction risk of CIN3 compared to a 51 percent reduction for women vaccinated at age 17.
The researchers conclude that the study demonstrates that routine immunization with the HPV vaccine is highly effective against high grade cervical disease.
